[Early enteral infussions in complex treatment of severe combined chest trauma].

On the basis of study and treatment of 98 victims it was stated that the absorption of isotonic glucose electrolytes solutions in the small intestine was saved even increased in case of large hemorrhage in severe polytrauma with chest, abdominal injury and other regions trauma. An application of early enteral infusions allowed reducing of the volume of parenteral introduction solutions and facilitated to a reliable decrease of development frequency of nosocomial pneumonia of attributive lethality.

[Early enteral therapy and nutrition maintenance of patients with colorectal cancer complicated by acute intestinal obstruction].

An original authors' method takes into account the specificity of this category of patients, initial malnutrition, intestinal obstruction and proposes early enteral therapy and nutritional care. The use of the method in complex treatment of tumor intestinal obstruction improves the indices of the trophological status, decreases the degree of endogenous intoxication and number of postoperative complications, lethality and time of hospital stay.

It was investigated peculiarities of the constitutional malnutrition in 94 young men. The influence of oral supplementation by enteral nutrition on recovering 54 young men with constitutional malnutrition was examined. The results of investigation indicated a high efficacy of using oral supplementation in the complex treatment of malnourished young men. It results in improvement of patient's rehabilitation and reduce the time of hospitalization.

There are no standards of nutritional support of wounded patients in different periods of disease. Influence of supplementation with Nutrition on dynamic of anthropometric indexes, some biochemical and laboratory parameters in wounded malnourished patients in the last period of disease was examined. The results of investigation indicated that supplementation with Nutrizon increased all tianthropometric indexes, normalized albumin blood level, nitrogen balance, decreased nitrogen of urine.

Main stages of artificial nutritional support (ANS) in the surgical clinic are considered as a scientific system of diagnostic and curative measures. Principle aspects of trophological screening of the surgical patient are elucidated. The algorithm of choice of the principle of ANS is presented: parenteral and enteral. Grounds are given to confirm the priority of the enteral method of ANS at the main stages of managing the surgical patient: before operation, during it, at the early and late postoperative periods.

It considers its purpose, tasks, and areas under study and gives the results of some original studies of the gastroduodenal and hepatobiliary systems and the small intestine. Special emphasis is laid on new guidelines to examining the functional and morphological states of the alimentary tract.

The author studies new approaches to the ulcer disease classification conducted in the interests of medical rating board. It is proposed to distinguish acute and chronic forms of this illness in order to diagnose the real prior disease and to determine the category of patients in which ulcer disease was not diagnosed at the preceding stages of medical care. It is reasonable to classify four course phases of illness instead of two, and determine the gravity of ulcer disease which will predetermine medical and rating tactics concerning these patients. In the author's opinion it is necessary to include clinical variants of this disease into classification chart to improve the early diagnosis of recidivations.

The health of a man is predetermined to a considerable extent by his nutrition. Army physicians frequently deals with the cases of insufficient nutrition among servicemen. The article studies the reasons, frequency and clinic aspects of insufficient nutrition. The authors give a historical reference about the notion of nutritional dystrophy, and reveals some mechanisms of its development. These considerations concerning the cases of insufficient nutrition in servicemen will help army physician to carry out a more effective preventive work among military contingency.

The article makes a definition of the "trophological status" and describes various methods of its estimation which could be successfully applied for an early diagnosis of trophological insufficiency in servicemen.

It is recommended that a standard ration will be accompanied by "Unpit", natural casein concentrates, and "Glucosolan" vitaminized preparation to eliminate++ the disorders of trophological status of minimally wounded. There is a proposal to include the products of enteral nutrition for the ration of food supplies, because it would shorten the recovery time of minimally wounded, and, thus, accelerate their return to the ranks.

Two groups of pathogenetic mucosal changes peculiar to fasting period were revealed in 28 obese patients: a structural rearrangement of mucosa with inhibition of mitotic activity of epithelial crypts, and the inflammatory responses manifested by increased cellular infiltration of stroma and compensatory-adaptive changes of capillary bed structures. The adequate dietotherapy provided full recovery of the mucosa tissue parameters on the 26th day.
